{"id":2739,"date":"2025-08-26T15:44:00","date_gmt":"2025-08-26T14:44:00","guid":{"rendered":"https:\/\/staff.nhslothian.scot\/ahp\/?page_id=2739"},"modified":"2025-08-26T15:45:51","modified_gmt":"2025-08-26T14:45:51","slug":"prbl-student-datix","status":"publish","type":"page","link":"https:\/\/staff.nhslothian.scot\/ahp\/ahp-practice-based-learning\/ahp-prbl-governance-and-assurance\/prbl-student-datix\/","title":{"rendered":"PrBL: Student Datix"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">Datix Reporting<\/h2>\n\n\n\n<p>Any AHP student involved in an incident and\/or adverse event should complete the Datix form with support or be present during the completion of the Datix form as part of the learning&nbsp;process.<\/p>\n\n\n\n<ul style=\"padding-right:var(--wp--preset--spacing--60);padding-left:var(--wp--preset--spacing--60)\" class=\"wp-block-list\">\n<li>The practice educator and\/or the student coordinator as well as the student\u2019s HEI must be informed of any incident or adverse event involving a student, whether directly or indirectly.<\/li>\n\n\n\n<li>The AHP student and\/or practice educator should inform the Academic Tutor&nbsp;of&nbsp;the incident\/ adverse event&nbsp;as&nbsp;soon as possible and within 72 hours of the incident\/ adverse event occurring.<\/li>\n\n\n\n<li>The Academic tutor&nbsp;will contact&nbsp;the student, as per HEI policies, as soon as possible but no more than 5 working days of being informed of the incident&nbsp;to identify any additional learning or developmental needs. Response time will be proportional to the severity of the incident.&nbsp;<\/li>\n\n\n\n<li>The Academic tutor&nbsp;will&nbsp;liaise with the student coordinator\/team lead\/operational manager and practice educator&nbsp;and inform them of any learning or developmental needs identified to support the student and support the implementation of these if required.&nbsp;&nbsp;<\/li>\n\n\n\n<li>The&nbsp;practice educator\/student coordinator\/team lead\/operational&nbsp;manager will make referrals as appropriate as per NHS Lothian policies and&nbsp;as a result of the investigation into the incident.<\/li>\n\n\n\n<li>The&nbsp;practice educator\/student coordinator\/team lead\/operational manager will&nbsp;complete the Datix review as per NHS Lothian policy.&nbsp;<\/li>\n\n\n\n<li>The AHP PEL receives notification of all Datix that mention \u2018AHP student\u2019&nbsp;and these&nbsp;are reviewed by the AHP PEL Team&nbsp;within 72 hours of the Datix report being submitted.&nbsp;As appropriate, the AHP PE Team will make contact with the student coordinator\/team lead\/operational manager. This is to ensure awareness of the incident\/adverse event, appropriate communication with the university and to offer support if required to student&nbsp;and staff involved<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-group has-light-accent-grey-background-color has-background\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p style=\"padding-right:var(--wp--preset--spacing--70);padding-left:var(--wp--preset--spacing--70)\">Types of events that may be reported include, but are not limited to:<\/p>\n\n\n\n<ul style=\"margin-right:var(--wp--preset--spacing--60);margin-left:var(--wp--preset--spacing--60);padding-right:0;padding-left:0\" class=\"wp-block-list\">\n<li>severe harm<\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">verbal or physical abuse&nbsp;<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">exposure to blood or body fluids&nbsp;<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">inappropriate behaviour in a digital environment<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">exposure to radiation&nbsp;<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">needle stick injury&nbsp;<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">significant injury involving sharps&nbsp;<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">failure of a medical device or therapy equipment causing harm<\/span><\/li>\n\n\n\n<li><span style=\"letter-spacing: -0.1px\">exposure to occupational disease&nbsp;<\/span><\/li>\n\n\n\n<li>a breach of data protection regulations<\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<p class=\"has-light-accent-grey-background-color has-background\"><strong>The AHP PEL Team will collate information bi-annually to identify themes, number of incidents in each profession and potential learning opportunities to be shared at the AHP
