{"id":511,"date":"2025-08-22T09:35:00","date_gmt":"2025-08-22T08:35:00","guid":{"rendered":"https:\/\/staff.nhslothian.scot\/falls\/?page_id=511"},"modified":"2025-08-22T09:36:27","modified_gmt":"2025-08-22T08:36:27","slug":"edinburgh","status":"publish","type":"page","link":"https:\/\/staff.nhslothian.scot\/falls\/edinburgh\/","title":{"rendered":"Edinburgh"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">People living in the&nbsp;community<\/h2>\n\n\n\n<p>If you are looking to&nbsp;signpost&nbsp;a person living in the community to local information that can help reduce their risk of falls, you can use some of the links in the Edinburgh Falls Prevention Pathway, or direct them to this page:&nbsp;<a href=\"https:\/\/services.nhslothian.scot\/fallssupport\/i-live-in-edinburgh\/\">I Live in Edinburgh \u2013 Falls Support<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Multifactorial&nbsp;Falls Assessment and Rehabilitation at Home&nbsp;<\/h2>\n\n\n\n<p>For patients who would benefit from a\u00a0multifactorial\u00a0falls assessment\u00a0at home, a referral can be made to\u00a0<a href=\"https:\/\/weare.nhslothian.scot\/ecps\/edinburgh-hub-services\/\" target=\"_blank\" rel=\"noreferrer noopener\">Edinburgh Hub Therapy\u00a0Services<\/a>\u00a0via TRAK using a\u00a0<a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2024\/02\/EFARSRequestServiceGuide.pdf\">Request for Service<\/a>\u00a0or call\u00a0<a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a>\u00a0on the\u00a0clinician-only falls\u00a0telephone numbers 0131 200 2338 \/ 4262.\u00a0\u00a0Referrals are organised into urgent and non-urgent. A\u00a0practitioner working within a therapy team will aim to respond to urgent referrals within 24 hours and organise a visit within 48 hours (except on weekends). Please allow for 7-10 working days for non-urgent responses.<\/p>\n\n\n\n<p>The practitioner will work with the patients to carry out a multifactorial assessment of both the person and their home&nbsp;environment for falls risks. They will advise and support with an individualised&nbsp;balance, strength, gait and functional rehabilitation programme. They can also arrange provision of equipment or&nbsp;minor adaptations to improve independence with daily activities and complete onward referrals to further reduce falls risk.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Risk of Admission<\/h2>\n\n\n\n<p>If the patient is at&nbsp;imminent risk of hospital admission&nbsp;due to falls&nbsp;within the next 4 hours&nbsp;follow the&nbsp;<a href=\"https:\/\/apps.nhslothian.scot\/refhelp\/guidelines\/urgenttherapyandsocialcare\/\" target=\"_blank\" rel=\"noreferrer noopener\">Urgent Therapy and Social Care pathway<\/a>&nbsp;or <a href=\"https:\/\/staff.nhslothian.scot\/falls\/edinburgh-hscp-flow-centre-pathways\/\" data-type=\"attachment\" data-id=\"1008\">Edinburgh Flow Centre Pathway<\/a> by checking the referral criteria. Call&nbsp;the&nbsp;<a href=\"http:\/\/intranet.lothian.scot.nhs.uk\/Directory\/flowcentre\/Pages\/default.aspx\" data-type=\"link\" data-id=\"http:\/\/intranet.lothian.scot.nhs.uk\/Directory\/flowcentre\/Pages\/default.aspx\">Flow Centre<\/a>&nbsp;8:30am to 2pm&nbsp;on 0300 013 4000&nbsp;option 1 and 4, or&nbsp;<a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a>&nbsp;on 0131 200 2324&nbsp;out with these times.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Assistance to Get Up Off the Floor<\/h2>\n\n\n\n<p>For patients who are&nbsp;uninjured but stuck on the floor, follow the&nbsp;<a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2024\/02\/EFUPP.pdf\">fallen uninjured person pathway<\/a>&nbsp;to access urgent support to help them off the floor.&nbsp;This service is&nbsp;provided by the&nbsp;Assistive Technology Enabled Care&nbsp;(ATEC 24)&nbsp;monitoring and response&nbsp;team&nbsp;on behalf of the Edinburgh Health and Social Care Partnership.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Edinburgh Falls Prevention Pathway&nbsp;&nbsp;<\/h2>\n\n\n\n<p>If&nbsp;you&nbsp;are working with a person of any adult age living in the community who has had recent falls, worsening or increasing falls, or is at risk of falls, you can&nbsp;support&nbsp;them by following the<a href=\"http:\/\/intranet.lothian.scot.nhs.uk\/Directory\/FallsPrevention\/Community\/Documents\/Edinburgh%20Falls%20Prevention%20Pathway%20Healthcare%20professionals%20-%20JUN2022%20FINAL%20-%20Version%2018.pdf\">&nbsp;<\/a><a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2025\/08\/2024-02-09-EFPP-HCP-PDF-V21-JAN-2024.pdf\" data-type=\"attachment\" data-id=\"1022\">Edinburgh Falls Prevention Pathway V21 Jan 2024<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Movement, mobility<\/strong> and the home<\/h2>\n\n\n\n<p>Balance is important to reduce falls risk and for some individuals a walking aid can help. If the individual lives in Edinburgh, they can <a href=\"https:\/\/services.nhslothian.scot\/ecps\/self-referral\/\" target=\"_blank\" rel=\"noreferrer noopener\">self-refer for a walking aid assessment.<\/a><\/p>\n\n\n\n<p>Alternatively, you can be referred or self-refer for a walking aid assessment at the <a href=\"https:\/\/www.smart.scot.nhs.uk\/service\/disabled-living-centre\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Disabled Living Centre<\/strong><\/a>.<\/p>\n\n\n\n<p>There are often ways to make their home safer. You can use this guide to <a href=\"https:\/\/www.nhsinform.scot\/healthy-living\/preventing-falls\/home-safety\/prevent-falls-by-identifying-hazards-at-home\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>prevent falls by identifying falls at home<\/strong><\/a>.<\/p>\n\n\n\n<p>The individual may benefit from equipment to make the home safer and easier to live in. A health professional can assess and refer for this via <a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" data-type=\"link\" data-id=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\">Social Care Direct<\/a>. If they live in Edinburgh, they can <a href=\"https:\/\/www.edinburghhsc.scot\/livingindependently\/atec24\/communityequipment\/\" target=\"_blank\" rel=\"noreferrer noopener\">order community equipment and minor adaptations yourself<\/a>.<\/p>\n\n\n\n<p>Alternatively, the individual can request handrails and ramps from <a href=\"https:\/\/eur01.safelinks.protection.outlook.com\/?url=https%3A%2F%2Fwww.cre.scot%2Fwhat-we-do%2Feasiaccess-handrails-and-ramps%2F&amp;data=05%7C02%7Clibby.dale%40nhs.scot%7C51f6cef470a340e4b23c08ddaa940e41%7C10efe0bda0304bca809cb5e6745e499a%7C0%7C0%7C638854275197214299%7CUnknown%7CTWFpbGZsb3d8eyJFbXB0eU1hcGkiOnRydWUsIlYiOiIwLjAuMDAwMCIsIlAiOiJXaW4zMiIsIkFOIjoiTWFpbCIsIldUIjoyfQ%3D%3D%7C0%7C%7C%7C&amp;sdata=VS191ULh1KPKo9wMblz0wp4fwxEDADGhh6Tznn6ukRM%3D&amp;reserved=0\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Care and Repair<\/strong><\/a>. <\/p>\n\n\n\n<p>Joint pain and injuries can reduce mobility and function. Patients can be referred MSK physiotherapy through SCI Gateway and can also <a href=\"https:\/\/services.nhslothian.scot\/ecps\/self-referral\/\">self-refer<\/a> and use musculoskeletal self-help resources. via <a href=\"https:\/\/services.nhslothian.scot\/musculoskeletal\/self-help-information\/\" data-type=\"link\" data-id=\"https:\/\/services.nhslothian.scot\/musculoskeletal\/self-help-information\/\">NHS Lothian <\/a>and <a href=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/muscle-bone-and-joints\/\" data-type=\"link\" data-id=\"https:\/\/www.nhsinform.scot\/illnesses-and-conditions\/muscle-bone-and-joints\/\">NHS Inform<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Staying active to prevent falls<\/h2>\n\n\n\n<p>All adults or older adults are recommended to take at least 150 mins of exercise a week. This includes strength and balance exercises twice a week.   For more information, see the <a href=\"https:\/\/services.nhslothian.scot\/fallssupport\/wp-content\/uploads\/sites\/43\/2023\/05\/physical-activity-for-adults-and-older-adults-2019.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Physical Activity Benefits for Adults and Older Adults leaflet<\/a>. There are lots of health benefits that come from doing this including reducing the risk of falls.<\/p>\n\n\n\n<p>NHS Inform has information on <a href=\"https:\/\/www.nhsinform.scot\/healthy-living\/preventing-falls\/keeping-well\/strength-and-balance-exercises\" target=\"_blank\" rel=\"noreferrer noopener\">strength and balance exercises<\/a> suitable for doing at home. The <a href=\"https:\/\/www.nhsaaa.net\/wp-content\/uploads\/Super-6-exercises.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Super 6 Balance Challenge<\/strong><\/a> also has a range of six exercises that can help improve strength and balance.<\/p>\n\n\n\n<p>It is important that individuals have the choice of activities they enjoy. <a href=\"https:\/\/www.edinburghleisure.co.uk\/activities\/?page=2&amp;selected-age-range=23\">Edinburgh Leisure<\/a> has a wide range of activities to choose form.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.edinburghleisure.co.uk\/activities\/balanced-life\/\" data-type=\"link\" data-id=\"https:\/\/www.edinburghleisure.co.uk\/activities\/balanced-life\/\">Balanced Life<\/a> is a service provided by&nbsp;Edinburgh Leisure. It is an evidence-based falls prevention programme designed for adults who have had or are at risk of having a fall. Referrals by healthcare practitioners using this <a href=\"https:\/\/staff.nhslothian.scot\/falls\/wp-content\/uploads\/sites\/27\/2025\/07\/Balanced-Life-Referral-Info.docx\" data-type=\"attachment\" data-id=\"1017\">guide<\/a>, alongside this <a href=\"https:\/\/forms.edinburghleisure.co.uk\/edinburghleisure\/form\/ActiveCommunitiesBalancedLifeReferralForm\/formperma\/uymUbdMpg8rsX5niVwWM0RBw0iFGCw7o6JN13POaJzk\" data-type=\"link\" data-id=\"https:\/\/forms.edinburghleisure.co.uk\/edinburghleisure\/form\/ActiveCommunitiesBalancedLifeReferralForm\/formperma\/uymUbdMpg8rsX5niVwWM0RBw0iFGCw7o6JN13POaJzk\">referral portal<\/a>.  Each weekly session includes specific exercises designed to improve balance, endurance, strength and flexibility, alongside educational discussions. <\/p>\n\n\n\n<p><a href=\"https:\/\/www.edinburghleisure.co.uk\/activities\/be-active\/movement-for-memories\" target=\"_blank\" rel=\"noreferrer noopener\">Movement for Memories<\/a> is delivered by Edinburgh Leisure and is designed to support people with symptoms of dementia or memory loss to be active. Individuals can be referred to Movement for Memories by a healthcare practitioner, or can complete the online self-referral form.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.edinburghleisure.co.uk\/activities\/active-communities\/ageing-well\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Ageing Well<\/strong><\/a> is also delivered by Edinburgh Leisure in partnership with NHS Lothian, offers a range of accessible and enjoyable city-wide activities which support people to become and remain active in later life. All Ageing Well activities are subsidised by voluntary contributions, with some sessions being free and some at a small cost. <\/p>\n\n\n\n<p><a href=\"https:\/\/walkingscotland.org.uk\/\" target=\"_blank\" rel=\"noreferrer noopener\">Walking Scotland<\/a> is a Scottish charity that provides health walks in local areas. A referral can be made via&nbsp;<a href=\"https:\/\/www.edinburgh.gov.uk\/socialcaredirect\" target=\"_blank\" rel=\"noreferrer noopener\">Social Care Direct<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Community Equipment&nbsp;<\/h2>\n\n\n\n<p>For patients who would benefit from&nbsp;assessment&nbsp;for equipment to live independently, more support is available.&nbsp; For minor adaptations such as grabrails, handrails and bannisters, a self-referral can be made via the&nbsp;<a href=\"https:\/\/www.edinburgh.gov.uk\/orderequipmenthomeform\" target=\"_blank\" rel=\"noreferrer noopener\">online form<\/a>.&nbsp;
